Nomination Submission Guidelines
Nominations for the 2017 University and LAS awards for both faculty and staff are due in the LAS Dean’s Office by, December 15.
To ensure that nominations are given full consideration by the LAS Awards Committee, please read carefully through the nomination submission guidelines and the award criteria and eligibility. Each nomination for a college award must include an LAS nomination cover sheet, available in CyBox. These nomination cover sheets provide detailed information on the specific materials required in preparing the nomination. If you would like the nominee also to be considered for a parallel university award, you must complete a university award coversheet. Additional information on University Awards (faculty awards, staff awards) is available on the Provost’s website. University cover pages are available on the University Awards Nominations webpage. Note most nominations for faculty and staff awards can only be submitted by a college.
The LAS Awards Committee will review all nominations and select those to forward to the provost for the university award competition. Nominees must be Iowa State University employees and LAS faculty or staff members at the time of the annual Fall Convocation in order to be award recipients.
All nomination packets for LAS awards must include an LAS nomination cover sheet, availabe in CyBox. The cover sheets also provide descriptions, eligibility requirements, evaluation criteria, and required nomination materials for each award, as well as the relevant LAS nomination cover sheet in both PDF and DOC format.
The nomination must be submitted electronically via email to Nolan McDonald in the LAS College Office by Thursday, December 15. The electronic submission must be sent as an attached single file (either PDF or Word format) with nomination materials in the following order: (1) LAS nomination cover sheet; (2) university nomination cover sheet, if appropriate; (3) 100-word paragraph for convocation program; (4) nomination statement/letter and all supporting materials; (5) nominee’s curriculum vitae/resumé. The electronic nomination file should be named in the following manner: NOMINEELASTNAME_AWARDNAME. Please submit materials in the requested format and order to ensure timely review of the nomination.
All nominations that you wish to have considered for a parallel university award must also include a university nomination cover sheet:

- University Staff Awards
- University Staff Awards reviewed by P&S Awards committee
A single nomination may be submitted for both University and LAS awards, but the nomination must include both the relevant University and LAS cover sheets.
